Injector Pump Cold Start

On a cold start, if I pull the cold start knob out in my diesel van, it emits a lot of smoke and runs very rough while warming up. If I leave the knob in, the van starts normally and smoothly. This is my third diesel van and the other two did not do this. Timing is within specifications for the engine - injectors and glow plugs are good.

It's summer now (and I'm in California - yesterday was over 102 F), so starting with out the cold start doesn't seem to be a problem. However, when winter comes it does get cooler (into the 20's F) and I would like to have the benefit of the cold start operation. Any thoughts would be helpful.
